Get data from database and place it on printable form

Hi to all GC masters,

 

            I would like to ask help on how can I retrieve data from the database and place it on printable form.

   

             Example:

                          I have the table "application" with many fields like appid, LastName, FirstName, MI, AppiedFor, Price.

 

                          I want to input/search the "appid" then it will retrieve the data then display it on screen that I can print. Is it possible that I can manipulate the arrangement of the displayed fields?

 

 

             Can somebody help me about it? thanks!

OR

               Can I move or re-arrange the fields on the View option and  make it printable?
