Duplicated records when inserting

Hi All:

I mounted Codeigniter + twig + Grocery CRUD (last version 1.4.1). I have a problem inserting new records, I don't know why but when I use the add is inserting twice the row.

I checked all the JS and I think there's not duplicated files.

Someone had/solved the same issue?

P.S. : edit and delete works perfect.

I do have same issue facing , let me know if you get any solution for this.
